Summary
Calls for SJNCERT member registration, which allows us to: Better communicate with you; Create a fast and easy way to check in and check out of events; Allow you to participate in limited access exercises; Provide you with an official SJNCERT badge; Issue you gear that is not provided in your backpack; and Provide SJNCERT activity summary information – not including your personal information - to authorities.
1) All continuing members to register with SJNCERT using this short form *This form supersedes all other registration forms.
2) Since SJNCERT badges require your photo, please email sjncert.ellice@gmail.com with your headshot photo and your name, as it will appear on your badge. Photo is similar to Passport requirements. If you don’t have a headshot photo, we will be available at select SJNCERT events to take your photo.
Registering with SJNCERT allows us to:
- Better communicate with you
- Create a fast and easy way to check in and check out of events
- Allow you to participate in limited access exercises
- Provide you with an official SJNCERT badge
- Issue you gear that is not provided in your backpack
- Provide SJNCERT activity summary information – not including your personal information – to authorities.
Privacy Policy
The information you provide in the San Jose Neighborhood CERT Registration Form will be used only to contact you regarding SJNCERT activities. The information we collect is held in a protected, limited-access space. Without your written permission, we will not share your information with anyone, including other SJNCERT members. We will not share your information outside of SJNCERT channels. You may be contacted by your district SJNCERT leadership, SJNCERT.ORG leadership and administration team members, or the District SJNCERT Leadership Forum facilitator, currently David Burckhard, sjcertd8@gmail.com.
